Action Teams

Partners Ending Homelessness draws on the expertise of local community members to achieve the needs of multiple action teams critical to success of our work. If you are interested in learning more about one of the below action teams, please contact Jacqueline McCracken at 336.553.2715, ext. 101. These teams are working toward strengthening Guilford County's continuum of care and are essential to the actualization of our vision that Guilford County will be a community that ends homelessness. We encourage the participation of members of the faith community, business community, civic groups and the public to work together for change in our community.

Advocacy & Marketing Action Team

The Advocacy & Marketing Action Team develops strategies for educating the general public elected officials, the Faith community, business and civic groups about homelessness and related issues and develops communications with the media.

Assessment and Evaluation Committee

The Assessment and Evaluation Committee evaluates and determines the effectiveness of program outcomes; make recommendations for strategies to improve outcomes; and analyzes the needs and changes in Guilford County's homelessness delivery system. The Assessment and Evaluation Committee oversees the data collection & reporting committee and the granting committee.

Data Collection & Reporting Committee

The Date Collection & Reporting Committee is responsible for ensuring quality data collection and findings; overseeing implementation of HMIS; coordinating a HMIS users group; and conducting the annual Point in Time Count.

Granting Committee

The Granting Committee establishes priorities and policies and develops evaluation instruments; oversees an annual competitive process for funds that address homelessness in Guilford County; and coordinates the annual HUD Continuum of Care Homeless Assistance Program Notice of Funding Availability application process.

Resource Development Committee

The Resource Development Committee creates a fundraising plan and implements fundraising strategies to address homelessness and related areas in Guilford County.

Discharge Planning Action Team

The Discharge Planning Action Team works to ensure that every person being discharged from jail, hospitals, mental health care, or foster care has a discharge plan that leads to stable housing and supports a community policy of "zero tolerance" for discharge to homelessness.

Continuum of Care Action Team

The Continuum of Care Action Team works to increase awareness, access, and coordination of services within Guilford County's Continuum of Care.
